Big thanks to Vassili Bykov and Alan Knight for giving hints/ideas on how to get this to work. I'm running BottomFeeder. I'm also running VisualWorks IDE now. Simultaneously. At the same time. Cool.
Solution 1: open a terminal, cd to where your image is and execute:
open -a visual.app MyImage.im
The -a specifies the application to use.
Solution 2: Create a link to the VM app in the same directory as your images. Then just drag and drop the image onto that link. The VM app link is found in $VISUALWORKSHOME/bin/macx/visual. You can create the link in your image directory by drag and dropping the visual app there and dropping it with keys pressed.
For the mac/VW neophyte, you have to use the open command to run VisualWorks from the command line. If you just execute the executable found in the application directory directly, it will startup, but you won't get any keyboard input
