Chris Petrilli weighs in on the debate I was having here and on Aaron Swartz' blog, and sums up with:

James discusses the issue around hours, etc., but that’s much further down the consideration process than “I can’t do it because my kids need insurance” decision point.

To be honest, this is the kind of argument a person who is single without kids makes. Paying for health care out of pocket can be painful, but it's possible (I had to pay for a few drugs out of pocket this year when my insurance company balked - it was far less expensive than any trip I take to the dentist). You simply cannot create more time. As I said in the comments on my post: try explaining to the wife "Sorry dear, don't expect to see me for the next two years; I'll be busy".

There are people who do that, and they commonly end up divorced. It's just the way it is.
